Entering Spouse/Dependent Social Security Numbers in my.SMU

The IRS requires numerous data elements on the Form 1095-C, including the Social Security Number (SSN) of any individual covered by SMU’s medical plans. If you have not yet entered the SSN for an enrolled spouse and each enrolled child, as applicable, it is important that you do so immediately. 

Social Security Numbers are treated as strictly confidential and will only be shared with the IRS in accordance with ACA reporting requirements. 

1) Sign into my.SMU -> Employee Self-Service
2) Click Benefits -> Dependent/Beneficiary Info
3) Click the first name listed, scroll down, and click Edit
4) If SSN is not listed for that individual, enter it and click Save
5) Scroll down and click Return to Dependent/Beneficiary Summary
6) Repeat steps 2-4 for each remaining dependent
