Concentration Declaration Form – M.A.M. Degree
Students may choose to complete a customized concentration as part of the MAM 15 hour elective requirement. In order to complete a customized concentration, students must:
- Via this form, formally register for the concentration through the office of the Perkins Registrar, the concentration adviser, and the Perkins Associate Dean for Academic Affairs
- Have sufficient hours remaining in their degree program
- Not yet have applied for their internships
Note: M.A.M. students must register for a concentration no later than the first semester of study at Perkins.
