Panic Button Request Form

 

 

  1. Requesting person fills out the “Panic Button Request form,” click the begin button at the bottom .
  2. Request will go to an officer who will reach out to the requester to schedule a security assessment of the area.
  3. Once the assessment is complete, the officer will submit a help desk ticket with the recommendation that was discussed with the requester.
  4. OIT sets up either the panic button or other recommendation (i.e. silent dial from the phone).  Depending on the recommendation, there may be a cost associated with this installation.  Once complete, OIT will notify the police department to follow up with the requester and provide training on how to use and education on when to use.
