If you or your parents are self-employed, you must also write on the verification form the estimated Adjusted Gross Income (AGI) and amount of Taxes Paid (the amount that would appear on Line 55 of the 1040, Line 35 of the 1040A, or Line 10 of the 1040EZ) for the tax year in question. IRS Form 1040-ES may be helpful in determining the amount of Taxes Paid.
