Academic Initiatives (AI) Purchase Authorization Workflow – Web-based approval form for any Engage Dallas-related purchases. Authorization must be received prior to purchasing.

Common Curriculum Intent Form – Housed within the Engage Dallas: Community Engagement Proficiency Canvas Course, a DocuSign used by the Office of General Education to collect students’ intention to complete the Community Engagement Proficiency and Experience.

Engage Dallas General Engagement Event Submission Form – Official method to request an event count towards Engage Dallas general engagement hours for the CEPE. This process facilitates attendance tracking, advertisements, and communications between appropriate Engage Dallas partners.

Engage Dallas Community Partner Information Form- A webform seeking relevant Community Partner information to determine organization and mission alignment with Engage Dallas; this form must be completed to become an official Engage Dallas Community Partner.

Engage Dallas Individual Service Registration - A webform that allows students to register for individual service through Engage Dallas (coming soon); students will receive specific instructions on how to sign up for individual service based on the type of service (e.g., solo service, internship, or individual pathway) indicated on this form.
