MEDICAL TREATMENT FOR WORK RELATED INJURIES
Ø If you have an on the job, work related injury – you have 48 hours to report the injury to the Office of Risk Management using the First Report of Accident/Incident.
Ø Medical care for work related injuries is provided through the Liberty Health Care Network (The Network). The Network is certified as a health care network by the State of Texas.
The notification (click either English or Spanish link above) document tells you what you need to know about this program.
The Network’s Service Area includes the following counties: Austin, Brazoria, Chambers, Collin, Dallas, Denton, Fort Bend, Hardin, Harris, Hill, Montgomery, Rains, Rockwall, San Jacinto, Waller, and Wise.
You will have access to hospitals, specialists, and treating doctors who are available twenty-four (24) hours a day, seven days a week.
If you are found to live in the Service Area you must choose a treating doctor from the network.
If you do not live in a rural area:

your treating doctor or hospital will be no more than 30 miles.

access to specialist or specialty hospital will be no more than 75 miles.
If you think you live outside the Service Area:
-
tell your employer and call your Claims Case Handler for review.

you can receive care from the network during this review.

a reply will be sent within seven (7) days of receiving your request for review.
FOR WORK RELATED INJURIES:
ü A doctor is provided through Liberty Health Care Network.
ü Report the injury to the Office of Risk Management.
ü Complete the first report of accident/incident form within 48 hours
ü Contact Risk Management for nearest Liberty Health Care Network approved facility.
