APPLICATION REQUIREMENTS
Application form, 2 passport size photos, payment (money order or credit card form NO PERSONAL CHECKS) and a current/valid copy of passport (page(s) that include passport number, picture, identifiable information, and expiration date must be included with the application and mailed to:
