Supplemental Life Insurance
You may purchase Supplemental Group Life Insurance for yourself, your spouse/domestic partner, and your children. You may only elect coverage for your dependents if you choose Supplemental Life Insurance for yourself. Premiums are paid through after-tax payroll deductions.

Did You Know?
If you do not enroll your spouse and/or children during the 31 day enrollment period and you wish to add them later, you will be required to complete an Evidence of Insurability form that must be approved by Reliance Standard.
Summary of the Plan
You may choose Supplemental Life Insurance up to five times your annual base pay, up to a maximum of $750,000. You may purchase up to $50,000 coverage for your spouse and up to $10,000 coverage for each of your children.
You must complete an Evidence of Insurability (EOI) form for any coverage amount in excess of three times your annual salary or $500,000, whichever is less. The excess amount will not be effective until Reliance Standard approves your evidence of insurability. The EOI form may be found on the Other Insurance Programs section of the BenefitsU website.
