Your SMU benefits are important to your family’s well-being. It is critical that you follow the instructions and enroll within the required time period.

You can print this form and use it as you go through the module. Check the forms you need to print, complete, and submit. All of the forms are available on the SMU HR website. After completing the program you may go there to print the forms you need.

New Employees
If you are a newly eligible employee, you can complete and submit your enrollment forms as early as your first day of employment. ALL FORMS MUST BE RECEIVED IN THE DEPARTMENT OF HUMAN RESOURCES NO LATER THAN 31 DAYS FROM YOUR FIRST DAY OF EMPLOYMENT. Your coverage will begin on the day your forms are received. If you don’t enroll within the 31 day period you will have no coverage and will not have the opportunity to enroll until the next annual open enrollment.
Qualified Life Change Event
If you experience a Qualified Life Change Event (i.e., marriage/birth/adoption) during the year, you may make certain changes to your benefits. To make a change, you must complete a Benefits Change Form and deliver it to the Department of Human Resources within 31 days of the event. This will be discussed in greater detail later in this module.
Annual Open Enrollment
Each year during open enrollment, you will receive information regarding any changes to the current Benefits Program and will have an opportunity to make changes for the upcoming calendar year.
