Benefits Enrollment form off the HR website and fax the completed form
within 31
days of the event date to 214-768-2043.
5. MetLife administers the Dental Plan for SMU. The
contact number for employees
is 1-800-942-0854. The contact number for dentists
is 1-877-638-3379. The
MetLife group number is SMU or 302292.

Logon to
www.bcbstx.com to print out a temporary ID and request a new one. The BCBS
Plan/Group # is 090138. If you do not know your ID number call 877-768-2005.
7. You have 31 days from the effective date of a
Family Status Change or during annual
Open Enrollment.
8.
You may
enroll in the Retirement Plan or make changes at any time. However, upon
completion of one year of service with SMU and attainment of age 36 your
participation
becomes mandatory.
9. You contribute 5% of your base salary. SMU
contributes 8% of your base salary if you are
age 26 through age 40. The SMU
contribution will increase to 10% the pay period following
your 41st
birthday.
10.
Yes, if you have a CREF account with TIAA-CREF or Fidelity. You may also
transfer
assets from Vanguard to TIAA-CREF or Fidelity to fund a loan.
There is
no penalty
for borrowing against your retirement account, unless you default on
your loan. You
will have to pay ordinary tax plus 10% excise tax on your
outstanding loan balance.

You have 31 days from your hire date to enroll in Medical, Dental, Vision,
Flexible
Spending Accounts, HSA, AD& D and Supplemental Life and Long Term Care
Insurance coverage.

You can
change your address through
HR Self-Service in Access.SMU or complete the
change of address form and mail (P.O. Box 0232) or fax to Human Resources at
214-768-2043.
13.
Job Evaluation Forms need to be completed in the following cases:
Any time the duties
and
responsibilities of a position change, even if the job title will not be
changing; An
employee is being promoted; A vacant position has not been
evaluated within the past
two to three years.

Staff increases are effective on or around June 1. Exempt staff increases go
into effect
June 1. Non-exempt staff increases go into effect during
the pay period closest to
June 1. Faculty increases are effective August 1.

Pay
grades are ranges of pay assigned to each job title, based on market pay data,
internal pay data, and the University’s ability to pay. Each pay
range has a
minimum, midpoint, and a maximum. Most employees within a specific
job title
will be paid between the minimum and maximum of the range. Pay ranges

are listed for 30
business days. After you complete and submit your online application,
it goes
directly to the Search Committee. The Search Committee is responsible for
reviewing all submissions received and selecting the candidates they are
interested in
inviting to campus for an interview. If you are selected as a candidate, the Search
Committee will contact you directly to setup an interview
date and time. Please allow a
minimum of 14
business days after the position closes for the search committee to
contact
selected applicants.

Please
discuss the matter with your supervisor in an effort to reach a mutually
agreeable
conclusion. If such a result is not achieved, please
contact the Employee Relations
manager in the Department of Human Resources. If
you are a faculty member you are
urged to use the normal procedures for informal
resolution of complaints. First through
consultation with the department head,
if applicable, then through consultation with the
dean of the particular school,
and finally an appeal to the Provost's Office. If your
complaint/grievance is
related to harassment, sexual harassment, or discrimination,
please contact the
Director of the Office of Institutional Access and Equity.
21. The Department of Human Resources will send you
your total compensation package
as well as schedule you to attend new employee orientation.
Our new employee
orientation will provide you with an overview of the University
and supply you with the
tools necessary for you to successfully paint your
future at SMU! New Employee
Orientation is conducted in a one-day format from
8:30am-5:00 pm on reserved
Mondays during the year in the Business Services
Conference Room located
in the basement of Clements Hall.
