90 Day Reviews should be completed prior to an employee's 90th day of employment. 90 Day Performance Reviews are conducted for all regular (benefit eligible) staff members.
Introduction
The 90-Day performance evaluation process is designed to be supportive and constructive and to improve both personal performance and library service. It assists the employee and supervisor in identifying strengths and weaknesses in job performance, and provides structured feedback to new employees as they approach the end of their introductory period at the university.
Process
(90-day Review)
2. Supervisor fills out Section I and Employee fills out Section II simultaneously.
3. Supervisor and employee meet to discuss the performance evaluation. Training and development opportunities are identified, if necessary, to meet the stated goals. It is helpful for each party to draft goals prior to this meeting in order to facilitate the discussion.
4. Final goals are identified in Section III.
5. Supervisor combines earlier sections and fills out Section V with his/her summary.
6. Employee responds if desired.
7. The signed original goes to Bill Dworaczyk in the CUL Personnel Office; supervisors should make copies for themselves and the employee. Keep in mind you may need to complete your 90 Day Review early so that it has time to progress from CUL to Human Resources.
8. If a staff member’s 90-day review falls during the annual review period (January to April), then the 90-day review will also serve as the annual performance review. Pease include a memo indicating the review is for BOTH purposes.

10. If you wish to request an extension to the introductory period, immediately contact Rebecca Sampson at 8-4085 to discuss your request. Complete the review and submit it to the Department of Human Resources after discussing your extension request, but prior to the 90th day of employment.
11. If your employee IS NOT going to successfully complete the University's 90 day introductory period, then do not complete the performance review. Immediately contact Rebecca Sampson at 8-4085 for instructions regarding termination during the introductory period.
