CORE Survey Research Design Workshop Registration

Registration is required for the following workshops. The colloquia are free and open to the public.

Registration Fee & Deadlines

The registration fee for each workshop is $120 and may be paid by check, money order, MasterCard/Visa, or a purchase order. Payment is required to reserve a space in a workshop. If you are submitting a purchase order for payment, please include a copy with your completed registration form. The fee does not include lunch. Recommendations for local dining venues will be provided at the workshop. Registration opens January 6, 2014; please register no later than 1 week before each workshop. 

Registration Process

Click here to access the interactive registration form. Once completed, please print and submit it using one of the methods below. If you do not wish to complete the form, you may call and provide your information over the phone (4th option below). 

Four ways to submit your form!

Fax: Fax your completed registration form with a copy of your purchase order or a credit card # to 214-768-7714.

Mail: Mail the completed registration form with a check, a money order, credit card information, or a copy of your purchase order to SMU CORE Workshops, PO Box 750511, Dallas, TX 75206. Please keep a copy for your records.

E-mail: You may email a copy of your completed registration form to Attach a copy of a district purchase order or call 214-768-7715 to provide your credit card number. Unfortunately we are not permitted to accept credit cards by email due to identity protection issues.

Phone: Call 214-768-7715. Payment for phone registrations can only be made by credit card or a faxed/emailed copy of a purchase order.

Registration Confirmation: Once our staff has processed your registration, a confirmation will be sent to the e-mail address provided on your form. Pertinent course information, time/location changes, etc. will be sent to this address. You should receive an e-mail confirmation within one week of either faxing or mailing a registration; if you do not, please call 214-768-7715 to confirm your registration.

Cancellation Policy

Full refunds are made up to 7 days before the day of the workshop. If you must cancel your registration during 7 days prior to the workshop, a $25 processing fee will be withheld from your refund. You may, of course, send someone else (a substitute) in your place and avoid the cancellation fee. No refunds will be given after the day of the workshop. Workshops are subject to cancellation due to insufficient enrollment, but full refunds are given in these rare cases.
