ATTENTION STUDENTS:

The State of Texas has revised the Bacterial Meningitis Immunization Law (SB 1107) for new students.  For detailed information about SB 1107, click HERE.

The revised immunization law may apply to you IF:

  • You are under the age of 30; AND

  • You have not been vaccinated against Bacterial Meningitis within the past 5 years; AND

  • You are a new student beginning after January 1, 2012; OR

  • You are a continuing student that sat out the term prior to January 1, 2012

What you need to do:

·         Provide the SMU Health Center with proof of immunization against bacterial meningitis within the past 5 years and at least 10 days prior to the registration for each term.  CLICK HERE FOR THE FORM! See the Academic Calendar for dates.  (You may submit your form to the Dispute Resolution program administrative office to be forwarded to the Health Center, but allow a minimum of two or three extra days if you do this.)

·         Click HERE to download the HEALTH CERTIFICATE.

·         Click HERE to see a partial listing of where you can obtain a vaccine near the SMU in Plano campus.

Failure to provide proof of immunization (Validated Health Certificate) will result in a “hold” being placed on your record blocking enrollment of classes.

Students requesting exemption from obtaining the vaccination due to health issues or reasons of conscience:

·         Click HERE to complete the Affidavit Request for Exemption from Immunizations.

Visit the Health Center website at http://smu.edu/healthcenter for more information, including where to submit the certificates.
