This form is to be used by faculty, staff, and students of Perkins School of Theology only. Persons or groups from outside the Theology School should submit a Request for Use of Facilities Form in hard copy.

Room reservations to faculty and staff are made on a first-come, first-served basis. Perkins student groups must have a Perkins faculty or staff sponsor to use the facilities.  Please fill out the form completely. Once you click the 'Submit' button, your request will be transmitted to the Perkins Office of Public Affairs. You will receive a confirmation email message once your reservation has been completed.
