End-of-Course Evaluation Forms
The Perkins Intern Program is built on the premise that an internship is most successful when all members of the internship supervisory team—the intern, mentor pastor, lay teaching committee, and Intern faculty—work together to provide the student a rich and multi-dimensional learning experience.
The following end-of-course evaluation forms offer the members of the team an opportunity to look back over the internship year and to offer a candid assessment of the Intern Program and constructive suggestions to improve it, information that will help the Intern faculty and staff better serve future interns.
