Application Forms

The Engaged Learning project application requires the following:

The Project Information Form online  

The following forms to be submitted via Blackboard

Project Proposal
Reflection Exercise 1
Funding Request

It requires the mentor to write a strong letter of recommendation.

Mentor Recommendation Request 

If the project is hosted by a community partner, the application also requires that a representative from the organization write a letter of support.

Host Agency Letter of Support Request

Here's How It Works:

1. Complete the Project Information form.


2. Develop your proposal with your mentor and attach it to the Project Information form.


3. Complete Reflection Exercise 1 and add it to your packet.


4. Decide if you will need funding for your project. If so, complete the Funding Request.


5. Give your mentor the Mentor Recommendation Request. As him/her to write a letter as a part of your application


6. If you are working with a host agency, give the person in charge the Host Agency Letter of Support Request. Ask him/her to write a letter that shows support of your project.


7. Submit your complete application packet to the Office of Engaged Learning prior to the deadline.
