Performance Reviews are to be conducted on an annual basis for all staff members. The Department of Human Resources has established the spring semester, January to April as the Review period.
Performance Reviews are due to Human Resources by April 11, 2014.

Remember to forward the following documents:
- Original of The Expectations-Staff Member's Perspective
- Original of The Expectations-Supervisor's Perspective
- Signed and dated original of The Plan (may be incorporated with the Staff Member's form)
If your area has received approval to use a different form, please forward all original documentation.
