We offer two alternative screening options for employees who do not participate in the on-campus screening process. Once you've completed an alternative screening, you will also need to take the online Health Risk Questionnaire (HRQ).
Because the screening and HRQ are required by both Wellpower and by the Deductible Discount Program, there are several deadines to note. You must complete both the screening and the HRQ before the applicable deadline.
- To be eligible for a Wellpower Reward for the Fall 2012 Program: February 1, 2013
- To be eligible for a Wellpower Reward for the Spring 2013 Program: June 21, 2013
If you completed a screening and HRQ in the fall of 2012, you are eligible to earn a fall and spring reward and do not need to complete a screening/HRQ in the spring of 2013. You will need to complete another screening/HRQ in the fall of 2013.

Screening Options
Option 1: Using Your Own Physician
To have this option paid for at 100% by SMU, you will need to currently have coverage though one of the SMU Health and Wellness medical plans. Be sure to take your BCBSTX insurance card with you to the appointment as it will be billed to BCBSTX. You may call and schedule your screening when it’s convenient for you to do so
- Get an appointment with your medical provider for a physical
- Take the Physician’s form with you to your appointment (when you’re getting the results)
- Ask the provider to complete the form (you may need to take your own waist measurement)
- Fax both the form and a copy of your lab results to Baylor at the number on the form
- Complete the Health Risk Questionnaire (use your lab results to answer questions)
Option 2: Get Screened at Park Cities Dallas Diagnostic Association
To have this option paid for at 100% by SMU, you will need to currently have coverage though one of the SMU Health and Wellness medical plans. Be sure to take your BCBSTX insurance card with you to the appointment as it will be billed to BCBSTX. You may call and schedule your screening when it’s convenient for you to do so.
- Call the Dallas Diagnostic Association in Park Cities to make an appointment
- Be sure to fast after midnight before your screening
- Print out the Physician's form and take it with you
- Ask the screener to complete the form as much as possible (you might have to take your own waist measurement and enter it on the form)
- Get a copy of your lab work (they can fax or secure email it to you later)
- Complete the rest of the Physician’s form yourself
- Fax both the Physician’s form and a copy of the lab results to Baylor at the number on the form
- Complete the Health Risk Questionnaire (use your lab results to answer questions)
Health Risk Questionnaire (HRQ)
You must also complete the Health Risk Questionnaire by November 29, 2013 in addition to getting your screening. Have your lab results handy and give yourself about 20 minutes to complete the HRQ. If you're taking the HRQ for the first time, the eligibility ID# is your SMU ID# (all 8 digits) plus the letters: SMU.
