Using Results From Your Own Physician
You are welcome to visit your own physician to complete the screening/check-up requirements for Wellpower, the 2014 Deductible Discount Program, Naturally Slim and the new Healthy Outcomes pilot program. Click the link below for details.
Using Screening Results From Your Physician
Online Health Risk Questionnaire (HRQ)
As of August 1, 2013, the Online Health Risk Questionnaire through WebMD is no longer required to qualify for the deductible discount. In addition, the Wellpower program for 2013-14 will not require the HRQ through WebMD. However, in our new screening process, Catapult Health will complete a short Health Questionnaire during the onsite screening. If you choose to use screening results from your own physician, Catapult will email you a Health Questionnaire to complete via their secure email server. This Questionnaire is required to complete your screening.
Programs Requiring a Screening/Check-Up
The following programs each require submission of a biometric screening/health check-up by a specific deadline. Details about each program are available by clicking the appropriate link below.
If you complete a screening/check-up onsite in the fall of 2013, you meet the requirements for the 2014 BCBSTX Deductible Discount Program and Wellpower's Fall and Spring Reward periods. You may also elect to participate in the Healthy Outcomes Pilot Program and apply for the Naturally Slim program using the same results.
Put Your Screening Results To Work For You
We encourage you to complete a health screening every year. Share your results with your medical provider and finally, take advantage of the many resources offered through your benefits, the Wellpower program, and the University to improve your total wellbeing.
Your Private Health Information
And rest assured, the privacy of your medical and health information is very important to us. No individual health information obtained through these processes is ever shared with anyone at SMU. The University does receive a group, aggregate report describing the overall health of the participant group. This information is used to help make programmatic and educational decisions for the benefit of our participants. In addition to this report, the University may receive the names of people completing a screening/health check-up in order to process program-related incentives or validate that a program requirement has been met.
