This form is used to facilitate an agreement between an employee and their manager regarding adjusted work schedule for a specified period of time.
Assistance in completing the form may be obtained by emailing EmployeeRelations@smu.edu. Please include your name, smu id, and a summary of what you want to accomplish so that we may be prepared before we respond to you.
Once the form has been signed off on by both employee and manager, each should keep a copy of the form and the original should be submitted to the Department of Human Resources at PO Box 232, Attn: Employee Relations
