Download and complete the Information Sheet for Legal Documents/Contracts (Microsoft Word format) before submitting contracts to the Office of Legal Affairs.
Submit the completed form to the Legal Affairs attorney reviewing your document/contract.The Office of Legal Affairs will review the contract and forward the form and a letter requesting execution of the contract to the Office of the Vice President for Business and Finance.
